Do I need a referral to be seen at three forks nutrition?
I do not require referral from a physician to make an appointment. I do not bill insurance, however, I offer a Superbill to all of my clients who wish to submit their visits for reimbursement. It is the client's responsibility to determine if their payer requires a referral or authorization prior to being seen. See payment policy for more information.
