Payment for services is due at the time services are rendered, unless other arrangements have been made in advance.
Three Forks Nutrition LLC is not responsible for verifying your insurance coverage or for billing your insurance carrier on your behalf.
You will be asked to sign a consent to treat form at your initial visit. This is not only an agreement that you will pay for services rendered, but a covenant from me that I will uphold only the greatest standards for nutrition care for my clients.
